About
Care Design NY is a Health Home for individuals with Intellectual and/or Developmental Disabilities (I/DD). Our mission is to manage the care and coordinate disability services and supports, so individuals can live a quality life. Members and families are at the center of everything we do. We serve almost 28,000 individuals with I/DD in 30 New York State counties.
